Brain Fog Is Not a Diagnosis—It Is a Symptom

The Most Common Contributors After 40

  • Hormonal decline: Both estrogen and testosterone have documented effects on cognitive function. In women, the perimenopausal transition is frequently accompanied by self-reported cognitive difficulty, and research published in Menopause has confirmed that menopausal hormonal changes are associated with measurable differences in verbal memory and processing speed (Greendale et al., Menopause, 2009). In men, low testosterone has been associated with reduced cognitive performance in observational studies
  • Thyroid dysfunction: Even subclinical hypothyroidism—where TSH is mildly elevated but within the standard reference range—has been associated with cognitive impairment, including slowed processing speed and impaired executive function. A comprehensive thyroid evaluation including free T3, free T4, and reverse T3 may reveal dysfunction that a TSH-only screen would miss
  • Nutrient deficiency: Vitamin B12 deficiency is a well-documented cause of cognitive impairment, with neurological symptoms sometimes appearing before hematological changes. A comprehensive review in the New England Journal of Medicine described the neuropsychiatric manifestations of B12 deficiency including memory loss, confusion, and personality changes (Stabler, NEJM, 2013). Vitamin D, magnesium, iron, and omega-3 fatty acid status may also influence cognitive function
  • Insulin resistance: The brain is highly insulin-sensitive, and insulin resistance has been described as a contributor to cognitive decline—sometimes referred to as “type 3 diabetes” in the neuroscience literature. A study published in Neurology found that higher insulin resistance was associated with worse cognitive performance and accelerated cognitive decline in middle-aged adults (Willette et al., Neurology, 2015)
  • Chronic inflammation: Systemic inflammation—reflected in elevated hs-CRP, inflammatory cytokines, and oxidative stress markers—can impair neuronal function and has been associated with cognitive decline in longitudinal studies
  • Sleep disruption: Chronic sleep deprivation or fragmented sleep (common in perimenopause and in individuals with high cortisol) directly impairs memory consolidation, attention, and executive function
  • Environmental exposures: Heavy metals (particularly mercury and lead), mold exposure, and other environmental toxicants have been studied for their neurotoxic effects and may contribute to cognitive symptoms in exposed individuals
