This route is particularly appropriate for patients with documented malabsorption, those who have not responded to oral supplementation, patients with neurological symptoms (where rapid repletion is important), and individuals on medications known to impair B12 absorption.
A comprehensive review in Nature Reviews Disease Primers described the global prevalence of B12 deficiency and emphasized the importance of recognizing at-risk populations, including the elderly, vegetarians and vegans, patients with autoimmune conditions, and those with GI disease (Green et al., Nat Rev Dis Primers, 2017).
